How to Mix and Match Scrub Top and Pant Sizes
You shouldn’t have to buy a medium top and medium pants just because they’re sold as a set — most bodies aren’t the same size on top and bottom. The fix is simple: measure your top size and pant size separately, then buy from a brand whose sets let you choose each piece independently. The same-size set problem
Walk through scrub reviews for almost any brand and you’ll find the same story on repeat: the top fits beautifully, the pants cut into the waist — or the pants fit and the top billows like a sail. Nurses often report being a full size different between top and bottom, sometimes two. When a brand only sells matched-size sets, your options are all bad:
- Buy two full sets in different sizes and orphan half of each — doubling your cost.
- Buy separates instead and lose the set pricing.
- Compromise on one half of your body and spend a 12-hour shift regretting it.

Find your true top size
Three measurements decide your scrub top size, in this order:
- Chest: measure around the fullest part, arms relaxed. This is your anchor number against any size chart.
- Shoulder seam: the seam should sit at the edge of your shoulder, not sliding down your arm. A seam that hangs off the shoulder means the whole top will shift when you reach.
- Length: the hem should cover your waistband with room to spare, so nothing rides up when you lean over a patient or reach across a tray setup.
The reach test
Once a top is on, raise both arms overhead as if pulling supplies from a top shelf. If the hem clears your waistband entirely, size up or look for a longer cut. You do this motion dozens of times a shift; the top has to survive it.
Find your true pant size
Pants are decided by your hip measurement first, waist second — the opposite of how most people shop. A drawstring or elastic waist can cinch down to fit a smaller waist, but nothing can add room through the hips and seat once the fabric runs out.
The squat test
Sit-to-stand, drop to chairside height, squat to a bottom drawer, kneel to a low kennel: your pants live through hundreds of these cycles per shift. In the fitting room (or your living room), do one full deep squat. If the fabric pulls hard across the seat or the waistband drags down in back, go up a size in the pant only — not in the whole set.
Check the rise
If the crotch seam hangs low, the pant will restrict your stride; if it cuts high, sitting becomes the enemy. Rise problems usually mean the wrong size or the wrong silhouette, not the wrong brand.
